Medical Records / Health Information
This Medical Records Department is responsible for assuring documentation of care given to each patient. It maintains each patient's medical record to be easily retrieved by physicians as needed for future use.

Copies of Reports, Medical Records or Radiology Films
These can be obtained by the patient upon request. Patients must provide picture identification (driver’s license preferred) for proof of identification and sign a release of information form. Please allow 24 hours for records to be compiled.
Release of patient records is only done with the permission of the patient. Only the patient can authorize release of information, unless the patient is under 18 years of age. If the patient is currently less than 18 years old, his parent or guardian must sign the authorization to release the medical records.
If doctors are requesting records for continued care, there is no cost.
* There may be a fee if additional copies of records are directly requested by the patient.
The Medical Records Department is located on the lower floor of the hospital. Enter through the main Washington St. entrance. Go past the Information Desk/Registration Area and continue straight down the hallway to the end. Take a left by the Pharmacy go down past stairway and elevators. The Medical Records Department will be on your right, across from the Dining Room.
Department hours are Monday thru Friday, 8:00 am to 8:00 pm.

Medical Reports in an Emergency —please contact the House Supervisor at ext. 31530.
Patient records from a visit to the Tony Balice Clinic, Surgical Services Office, can be requested by calling the office at 616.523.1630. Medical records from a visit to the Tony Balice Clinic Family Healthcare can be requested by calling 616.523.1600. Please allow 24 hour notice for records to be compiled and copied. Records can be picked up at the requesting office, 550 E. Washington St., Family Health Center Building, Ionia, Michigan 48846.
